A journey through care
Dementia support · Bury St Edmunds
After a difficult stay in hospital, Margaret's family were told a care home was the only option left open to them.
Margaret wanted to stay in her own home, but her family needed to know she'd be safe, and consistently looked after, without a revolving door of unfamiliar faces.
We built a small team of three carers around her existing routine, rather than asking her to fit around a rota, and kept in regular touch with her GP as her needs changed.
Eighteen months on, Margaret is still safely at home, with the same small team supporting her day to day.

End of life · Stowmarket
Frank wanted to be at home with his wife rather than in a hospital or hospice as his health declined.
We worked alongside the district nursing team, covered overnight sits, and made sure Frank's family had respite and emotional support too, with an end-of-life trained team throughout.
Outcome
Frank passed peacefully at home, surrounded by the people he loved.

Hospital discharge · West Suffolk
West Suffolk Hospital referred Beryl for a home care assessment on a Friday afternoon, with discharge needed quickly.
We completed the assessment that same day, arranged medication support, and liaised directly with the ward so nothing was missed before Beryl went home.
Outcome
Beryl was home safely by Saturday evening, less than 48 hours after referral.
